Deadline for submission is April 1, 2008.

Event Registration Form Use this form
to register your events! This form will email the registration to
the Center for Fraternity and Sorority Life when the submit button is pressed.

Continuous
Open Bidding Form
This PDF form will print 2 copies of the Continuous Open Bidding Acceptance
Binding Agreement per page. Return one form per new member to the Panhellenic Office
in CUB 323 within 24 hours of signing.

Fraternity New Member/Associate Card (Red Cards)
This PDF form will print a master copy of the New Member Card. Return one completed card for each new/associate member ot the Center for Fraternity and Sorority Life in CUB 315 within two weeks of signing.

Authorization
to Move Out Form
Please complete this PDF form prior to moving from a Residence Hall to
a Chapter House.
Turn in
this form and your crimson or gray parking permit to the Hall Desk when
you move out of the Residence Hall.

Fraternity
Monthly Membership Report Form
This report is to be completed and turned in to the Center for Fraternity and Sorority Life on the 1st of each month. All information should reflect
changes in membership since the previous report. Be sure to attach
a membership roster to the form.

Fraternal Organization Agreement
Each organization must sign this document and submit it to the Student
Affairs Office in order to be recognized by the University. This PDFpacket includes a brief history of the FOA and of the Greek Alliance.

Crisis Management Protocol
This form lets us know who to contact in an emergency. It must be updated every semester and turned in to the Center for Fraternity and Sorority Life.

Report Form
This form should be used when you want to report an issue or concern
to the Center for Fraternity and Sorority Life, but do not intend for official conduct action
to be taken. If you would prefer conduct action to be taken, please
contact the Center for Fraternity and Soroirty LIfe for an Incident Report Form.
